Meeting the needs of the underserved
Texas has the highest percentage of uninsured residents in the nation, and in North Texas, thousands of individuals and families face the harsh reality of living without access to consistent, quality healthcare. Many are battling chronic diseases—diabetes, hypertension, COPD—without the guidance of a primary care provider. For uninsured patients, the emergency room often becomes the only option, leading to devastating health complications, financial instability and an endless cycle of crisis care.
Compassionate care, proven solutions
Through a targeted solution that has demonstrated success since its inception in 2007, Baylor Scott & White Community Care Clinics are helping break this cycle by providing proactive care to give people the tools to stay well—especially important for patients with one or more chronic conditions.
With four clinics in southern Dallas and additional sites in Fort Worth, Irving and Plano, BSW Community Care Clinics serve over 14,000 patients each year who are living at or below 200% of the Federal Poverty Level.
The clinics’ integrated care model —including primary care providers, social workers, pharmacists and community health workers—has reduced emergency department visits and hospital costs by 46% among engaged patients.
However, many still lack the specialty care, behavioral health services and social support needed to truly thrive. The opportunity is clear: sustaining and expanding access to these critical services will allow BSW Community Care Clinics to deepen its impact and transform even more lives.
